Alexander Beswick shone an animated blue NHS logo complete with a heart for the whole city to see.
The image was around 100-foot wide and 50-foot high and was projected from near Bass’s Rec.
Mr Beswick, who runs Empire AV Works in Derby, has carried out similar stunts in the past.
In August last year the trickster projected a 50ft-high image of male genitalia onto the side of the Jury’s Inn hotel in Derby city centre.
